Clinical Rotations

  • Breast Surgery: 36 weeks
  • Breast Imaging: 4 weeks
  • Pathology: 1 week + weekly pathology slide review
  • Medical Oncology/Radiation Oncology: 7 weeks
  • Plastic Surgery: 4 weeks

Breast Imaging Rotation

Four weeks are spent in the Radiology department, including hands-on experience with mammography, contrast-enhanced mammography, breast ultrasound, and breast MRI. This time is spent understanding the techniques of diagnostic mammography and BI-RADS nomenclature, as well as, identifying the imaging characteristics of benign and malignant disease. The fellow has experience performing breast sonography and image-guided biopsies. In addition, the fellow will complete the ACS or equivalent courses for basic and advanced breast ultrasound. 

Breast Surgery Rotation

Thirty-six weeks are spent on rotation with the breast surgeons. While on the surgical service, the fellow is responsible for pre and post-operative care of breast surgery patients. The fellow actively participates in the outpatient clinics, including the benign breast and genetic counseling clinics. The rotation schedule ensures the fellow time working with each of the breast surgeons. The fellow learns to evaluate and manage benign and malignant breast conditions.  They become proficient in the use of common in-office procedures such as breast sonography, cyst aspiration, FNA, and percutaneous core needle biopsy.

Pathology Rotation

One week is spent in the Pathology department evaluating benign and malignant breast specimens. The fellow is exposed to the techniques of fine needle aspiration of breast masses, marking and processing pathology specimens, immunohistochemical stains, cytology and tumor markers. In addition, the fellow reviews pathology for cases presented at Multidisciplinary Breast Tumor Board with the breast pathologist weekly during the 36 weeks of surgery rotations.

Medical Oncology/Radiation Oncology Rotation

Seven weeks are spent on rotation in the medical oncology and radiation oncology outpatient clinics focused on breast cancer. The fellow learns the indications, contraindications, mechanism of action, and common toxicities systemic therapies for breast cancer. The fellow learns to assess the indications and contraindications for radiation therapy as well as its complications.  Direct observation allows the fellow to describe the process of radiation therapy as well as the patients' experiences while undergoing radiation therapy. The fellow will receive a schedule of medical and radiation oncology case conferences and is expected to attend those pertinent to breast cancer care.

Plastic Surgery Rotation

Four weeks are spent on service with the Plastic and Reconstructive Surgery team. The fellow attends clinics to learn about the decision making process for various breast reconstruction procedures. The fellow participates in surgeries to learn the oncoplastic techniques of breast conservation, breast augmentation, and breast reduction.  For breast reconstruction, the fellow participates in cases to learn about the use of tissue expanders, implants, flap reconstruction and gender-affirming approaches to breast and chest wall reconstruction. The fellow may also participate in breast reconstructive surgeries while on the breast surgery rotation.
